I must take daily pain and arthritis medication daily to treat
a condition. I suffer constantly from gouty arthritis in my
ankles, feet and legs due to a condition caused by my
kidneys failing to eliminate uric acid from my system.
I suffer from constant gastro-intestinal disorders for which I
must take medication every day.
I have some weird skin disorders and blotches scattered
around my face.
I have recently developed some sort of cyst in my left hand,

I was diagnosed with a pliomorphic sarcoma I have had surgery
and it was sucessfully removed from my leg. I also had an
acoustic neuroma nine years ago. I had a daughter that at age
2 years 10 months was dianosed with Leukemia-she is now 22 |

| Illnesses: | While in the military I had epeditimitus, a condition that lasted
me a couple of years, this is when the eppiditimitus cord
leading to the testicle swells (right side) In 1988 probably a
couple months after getting out of the military and at end of
year, I got double phnemonia which I was in the hospital for,
for about 5 days. Now, I have severe muscle stiffness which I
have had for probably a total of 11 or 12 years; it has effected
my lifestyle for the last 9, I had to very much limit my phyical
activities for the last 9 years because of this severe muscle
stiffness restriction. I have had internal bleeding hemmroids
now for the last two or three years; it is managable though and
do not bleed to often or too much. My doctor also told me
recently that I have high cholesterol now; too much of bad
cholesterol, not enough of good cholesterol. I had my appendix
taken out when I was 33 years old, about 12 years ago, doctor
said I had a hard piece of stool that perforated appendix. |
